JOB REQUIREMENTS: Duties Valor Point (9A) is located within theCommunity Living Center is a 40-bed unit consisting of 2 programs,Transitional Care and Comprehensive Integrated Inpatient RehabilitationProgram (CIIRP). Transitional Care is a short stay (less than 90 day)interdisciplinary program which serves medically and psychiatricallystable veterans and provides nursing and medical care prior to theirreturn to a community setting. This program provides wound care/healingIV therapy/TPN, various orthopedic diagnoses, respiratory care; veteranand family education and veterans waiting title 19 approval andguardianship. The Comprehensive Integrated Inpatient RehabilitationProgram is an acute care inpatient rehabilitation program providingcomprehensive care by an interdisciplinary team to maximize function andabilities. This program provides an opportunity for appropriate veteransto improve deficits in mobility, self-care or safety concerns withattainable goals. Length of stay is determined by goals of care. Nursingcare provided is veteran focused and uses a team model.
