　　Job Summary:

　　Assists in planning and implementing nursing care services as per NJ Dept, of Health State and Federal regulations,

　　Provide continuity of care and services for each resident under the direction of the Nurse Manager, Charge Nurse and or Physician

　　Assesses resident needs on admission and as needed on a timely basis

　　Performs treatments and administers medication with physician s order

　　Responsible to follow standards of practice, policies and procedures set by facility, State, and Federal standards

　　Supervision of ancillary personnel in units to ensure quality of care services,

　　Participates in performance appraisals,

　　Functions as Charge Nurse if situation requires,

　　Assists ancillary staff with direct patient care activities as needed bed baths, incontinent care, and feeding,

　　Responsible for nursing documentation on each resident including, but not limited to, care plans, summaries, and nurses notes

　　Assists Charge Nurse in recording and collecting data needed for continuity of care diabetic testing, intake and output, physician s overdue visits,

　　Care and maintenance of nursing equipment and supplies

　　Communicates with nursing and ancillary personnel to coordinate planning intervention processes and procedures as needed,

　　Observes and reports on patient status to Nurse Manager and/or Physician

　　Assists Nurse Manager, Charge Nurse in staff replacement within guidelines and policies set by the Administration,

　　Attends and participates in meetings related to resident care and unit management

　　Assists Charge Nurse in coordinating and implementing interdisciplinary care plans to meet resident needs

　　Participates in change of shift reports,

　　Assists in admission, transfer, and discharge of residents,

　　Reinforces basic nursing care concepts for ancillary nursing personnel

　　Identification of high-risk residents, who would develop pressure sores and timely implementation of preventive and supportive strategies for wound care,

　　Accurate identification and assessments of residents prone to develop sentinel events such as dehydration, fecal impaction, fracture

　　Assists in implementing and maintaining preventive strategies for infection control and following infection control policies and procedures,

　　Reinforces the teaching efforts of Nurse Manager Charge Nurse/and/or In Service Education Department for all nursing personnel

　　Assist the nurse manager in MDS care planning process to ensure continuation of care

　　Ensure and assist in order transcription and monthly recaps,

　　Assumes responsibility for own personal educational and developmental needs,

　　Educational Skills Required Degrees, Certifications, License,

　　Graduate from an Accredited School of Nursing: Current and active licensure as a Registered Nurse in the State of New Jersey

　　Required to have current certification via American Heart Association for Basic Life Support (BLS)

　　Maintain CEUs

　　A certificate for Cardiopulmonary Certification is required

　　Proven supervisory/management experience in the dynamic field of long-term care

　　Strong interpersonal and communications skills
