Overview

　　St. Luke’s Health System in Twin Falls is seeking Full Time RNs (1.0 FTE) to join our Ambulatory Centralized Clinical Resources team.

　　This role will perform in-basket support work remotely for identified clinicals from a centralized nursing team. The RNs will be supporting outpatient clinics within the Internal Medicine and Family Medicine areas. This will include addressing patient concerns, providing education, utilizing protocols, and support the clinic and patients through timely resolution of concerns through written and verbal communication. This includes coordination of patient experience, adherence to evidence-based practice, and implementation of system initiatives. A successful candidate is an individual that can problem solve with a system outlook, develop strong relationships with system partners, and work quickly to resolve concerns.

　　Schedule:

　　M-F, 4-5 shifts per week

　　Day shift, hours of operation for the department are generally 0800-1700

　　No holidays/weekends

　　Remote (Must live within Idaho)

　　Minimal Travel may be required depending on clinical location served

　　Responsibilities

　　Identifies patient needs and determines priorities for care through assessment, planning, intervention, and evaluation.

　　Develops, implements and coordinates individualized plan of care for patients assigned through initial and on-going assessments.

　　Initiates assessment based on the patient's problems or needs, the physician's medical diagnosis, and the functional health framework.

　　Performs therapeutic nursing interventions.

　　Manages care of patients.

　　Documents care based on standards of care and practice, patient acuity, and/or clinical priorities

　　Advocates for evidence-based practice care standards and supports adoption of evidence-based practices and innovations in care delivery.

　　Provides patient centric care in alignment with professional nursing practice model.

　　Other duties and responsibilities as assigned.

　　Qualifications

　　Education: Nursing Degree

　　RN's with less than 1 year of RN experience at the time of hire into the RN position must have graduated from an accredited nursing program (ACEN - Accreditation Commission of Education in Nursing, CCNE-Commission of Collegiate Nursing Education)

　　Experience: 1 years’ experience

　　Licenses/Certifications: Current RN licensure from the State of Practice
