Job Description:Responsible for executing all aspects of bed management andtransfer center operations in a safe and timely manner, in accordance withstandard operating procedures and regulatory requirements, scope isinclusive of patient access, placement, transfers and bed management. Usesclinical knowledge, critical thinking and communication skills to supportthe Command Center in providing a reliable, high-quality service that allowsreferrals, transfers, and other healthcare settings to be accepted in anefficient and timely manner.
Scope
Assesses, directs and coordinates transfers and admissions from physicianoffices, hospitals, and other healthcare settings to the most appropriateaccepting service, facility, and level of care.
Facilitates patient placement by assessing patient diagnosis, acuity andlocation, applicable risk factors and unit criteria/bed availability.
Collaborates with the facility departments, medical staff, nursingstaff, and leadership to promote appropriate, timely patient placement sothat patient care is not delayed.Collaborates with facility staff to identifyand communicate bed capacity issues.
Leverages capacity dashboards and other real-time indicators to predictfuture patient placement pressure points and participates in the execution ofsurge-capacity protocols; and initiating interventions, within scope ofpractice, to accelerate patient flow or other mitigating barriers to optimalthroughput.
Assists with all aspects of patient logistics to includepre-registration, transportation, and bed planning; collects unitspecific data, and information needed to access and place patientsappropriatel
Adheres to regulatory compliance with all Command Center operations,including Stark, EMTALA, and any other pertinent to Call Centeractivities. Maintains current knowledge of regulatory requirements impactingoperations of the Command Center.
Provides timely and accurate transfer and admission documentation.
Works collaboratively with ourproviders, by taking orders, and keystakeholdersin the development and maintenance of transfer and admissionpathways for our patients.
Delegates as needed to the Transfer Center Tech and EMS Liaison.
Promotes mission, vision, and values of SCL Health, and abides byservice behavior standards.
Performs other duties as assigned.
May be required to be placed on-call during a regularly scheduled shift.

Minimum Qualifications
Bachelor's degree of Nursing (BSN)Current Colorado RN licenseCurrent BLS certification endorsed by the American Heart AssociationPreferred Qualifications
Minimum 5years RN experience in either ED, ICU and/or 5 ye rs acute careinpatient nursingACLS. A specialty certification (CCRN, PCCN, MSRN, or CEN)Critical Care or ED background. Hospital Supervisor, Charge Nurse orprevious transfer center experience. Experience with hospital throughput orcapacity management.
