Position SummaryAs a member of Peter Munk Cardiac Centre team, the Registered Nurse, Clinical Coordinator participates in managing the Outpatient Cardiac Clinics patients before, during, and after visits both inside and outside of the treatment room.
DutiesProviding daily in-person support (Monday-Friday) in the cardiac clinics (5NU) to staff, patients, and collaboration with Nurse Manager, Nurse Practitioners, Physicians, technologists, other nurses, administrators, flow coordinators and receptionist
Triage of outpatient cardiac referrals of multiple subspecialties, coordination with 50+ staff cardiologists
Coordination of complex appointments with other departments
Administration of IV infusions and other treatments in Heart Function Day Unit, arrangement of therapy plans
VenipunctureandPhlebotomy
Providing data as requested pertaining to referrals, wait lists, treatment room, etc
Communicating with patients and families to review care plan, address and escalate concerns as needed to appropriate provider or department
Providing patients and families with education on topics such as, but not limited to signs and symptoms of heart failure and self-care management of various heart conditions
Provide education/training/mentorship to UHN colleagues and external facilities as appropriate
Appropriate documentation of medication administration, communication with patients and families, other nursing actions as needed
Participates in refinement of the PMCC Outpatient Cardiac Clinics processes and workflows as appropriate
Applies best practice guidelines to ensure that patients receive care according to provincial standards
Other duties as assigned
QualificationsCurrent registration with the College of Nurses of Ontario
Bachelor of Science in Nursing required
Current BCLS Certification
Minimum five (5) years Cardiac +/or Cardiac Surgery experience required
Critical Care or Coronary Care Certification preferred
Ambulatory experience preferred
Remote monitoring experience an asset
Proficiency in Microsoft Outlook, Microsoft Excel
Proficiency in peripheral intravenous (IV) placement, management; PICC line access and removal
Experience administering blood products
Experience working with community health providers preferred
Experience in change management
Strong organizational and time management skills
Excellent decision-making and problem-solving skills
Excellent interpersonal and customer service skills
Excellent verbal and written communication skills
Demonstrated ability to cultivate and build relationships
Strong track record of high-quality work and engagement is essential
Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities

Able to work collaboratively within a group setting and independently - required
