About Registered Nurse - Case Manager

　　Requirements

　　Must have acute hospital experience in CM and discharge planning.

　　Do not submit CM's with only SNF, REHAB or Hospice experience.

　　Must have prior travel experience.

　　Certification is not required.

　　2 or more years.

　　Utilization Review is a plus.

　　ACM or CCM Certification is a plus.

　　1 yr. minimum recent experience with discharge planning as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital setting.

　　Need to have acute inpatient case manager experience.

　　BLS.
