　　Responsibilities:

　　• Assist residents with daily living activities, such as dressing, bathing, and eating

　　• Monitor patients’ health status and report any changes to the nursing team

　　• Administer medications and treatments as directed by medical staff

　　• Help coordinate rehabilitation programs for patients to aid in their recovery

　　• Maintain accurate records of patient care and progress

　　• Ensure a safe and comfortable environment for residents

　　Shift:

　　Monday-Friday with rotating weekend and holiday!

　　6am-2pm OR 2pm-10pm

　　Requirements:

　　• Valid NYS LPN License

　　• Prior experience (6 months) in nursing home or rehabilitation setting

　　• Compassionate and patient-focused attitude

　　• Strong communication and teamwork skills

　　• Ability to handle challenging situations with empathy and professionalism
