Job Description/Essential Functions:
Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
Currently, we are seeking a full time Quality Improvement Specialist to work in our Behavioral Health Services Division.
This position receives direct supervision from the QI Manager and Director of Quality Improvement. This position may receive supervision from the Regional Director. This position is responsible for the following duties;
*Participate in Agency committees and other task groups as assigned.
*Conduct regular quality of documentation audits.
*Tracks incident/investigation reporting and documents trend analysis.
*Provides feedback and works with Directors/staff in the development of focused educational programs on results of auditing and monitoring activities to affected staff and physicians.
*Prepares agenda and takes minutes at the QI meetings. Arrange room reservation, order food (when applicable)and circulate notices for meetings and educational programs.
*Assists in the development and staff education covering the privacy of, and access to consumer health information in compliance with federal and state laws and Horizon House's information privacy practices.
*Maintain knowledge of applicability of state, federal and payer regulations.
Job Qualifications:
Bachelors degree preferred with a minimum of two(2) years of quality improvement experience or anycombination of experience, education or training that would provide the level of knowledge, skill andability required.
*One year of supervisory experience preferred
Proficient in Microsoft Office Suite (MS Office, Excel and forms application(s).
Knowledge of statistics, data collection, analysis and data presentation.
. Familiarity with electronic health records preferred.
Excellent interpersonal communication and problem-solving skills.
Familiarity with accreditation and/or state licensure review processes.
Excellent communication skills, both oral and written.
Ability to work independently and detail oriented.
Strong organizational analytical skills.
. Local travel required.
. Valid driver's license with and acceptable driving record required.
