About Public Health Nurse-Home Visitation

  JOB REQUIREMENTS: Bright Family Beginnings Provide screenings virtuallyand via home visits to women and their families eligible forevidence-based programs via the Family Foundations state grant Carry amaximum caseload of up to 25 families as defined by model program(s)Assist families in establishing goals and outcomes with educationalsupports Educate and provide guidance to parents on implementation ofproper developmental activities for their children Identify populationsnegatively affected by events and crisis and the conditions, systems,and structures that support these outcomes Refer clients to medical careand other community services Adhere to the nursing process and thefidelity of model program(s) Develop and maintain cooperative workingrelationships with first responder agencies, local community partners,and stakeholders Nursing Services Perform initial intake assessment andconduct appropriate health screenings for clients Aid in the assessment,coordination, planning, and evaluation of care plans and treatment goalsResearch and refer clients for additional medical care and communityservices and resources as needed Assist clients in enrolling in stateassistance programs Perform pregnancy testing, counseling, referral, andeducation Perform contraception counseling and management Prepare andadminister oral and parenteral medications for treatment of substanceuse disorders, sexually transmitted infections (STIs), and contraceptionmanagement, while abiding by the rights of medication administrationPerform a variety of other clinic services including but not limited toimmunization administration, genetic testing, childhood lead screenings,tuberculosis screenings, and family planning/reproductive health visitsAdhere to the nursing process and the fidelity of model program(s)Ensure adherence and compliance to the policies and procedures of thefederal Health Insurance Portability and Accountability Act (HIPAA)regarding protected health information Health Promotion and CommunityHealth Improvement Initiatives Provide education and community awarenesson behalf of Kenosha County Public Health while networking with otherpublic and private community groups, coalitions, and organizations(i.e., health related agencies, courts, schools, etc.) Participate inthe planning, development, and implementation of public health policies,procedures, programs, and services Collaborate with other public healthprograms and community organizations to strategically develop collectiveimpact strategies Participate in off-site mobile health missions in avariety of capacities Assist with identifying, developing, and advancingpublic health interventions to improve social determinants of health andhealth equity in our community Provide gender-inclusive sexual andreproductive health care services to the community through education,disease prevention interventions, and anticipatory guidance counselingParticipate in the prevention of STIs, HIV, and other communicablediseases through one-on-one counseling, group education, and communityoutreach Collaborate with other teams to provide education on theCommunity Health Improvement Plan (CHIP) and Strategic Plan DataCollection, Analysis, and Reporting Utilize various informationtechnology systems, including electronic medical records, internetdatabases, spreadsheets, and word processing programs, to collect dataand maintain a consistent database of client information and sessionnotes Provide analyzed statistical data at the county, state, andfederal level to meet reporting requirements for public health programsParticipate in ongoing community assessment which includes regular andsystematic collection, assembly, and analysis of community healthstatistics to identify community health needs and emerging trendsKenosha County is committed to Equity, Diversity and Inclusion and is anEqual Opportunity Employer. \ **** OTHER EXPERIENCE ANDQUALIFICATIONS: Experience At least one year of nursing experiencepreferred Success Factors Knowledge of: Principles and practices ofprofessional nursing Health promotion and health prevention Populationhealth practices and community health improvement Health equity, racialequity, and the social determinants of health Mental health conditionsand treatment options Health education behavior change theories and useSkill in: Critical thinking, systems thinking, analysis, andproblem-solving Time management and prioritization Oral and writtencommunication Use of Microsoft Office, including Word, Excel, Outlook,and PowerPoint;
