Job Description:The Clinical Psychologist is a vital member of amulti-disciplinary team composed of psychiatrists, psychologists,nursing, and other disciplines to provide holistic or comprehensive medicaland counseling services to patients. They interview patients and givediagnostic tests. Additionally, the Clinical Psychologist providesindividual, family or group psychotherapy, and collaborates with thebehavioral health team to develop and implement behavior modificationprograms, with treatment and intervention programs that patients canunderstand and comply with.
The Psychologist is responsible for providing outpatient psychotherapy topatients of Uptown Community Health Center, which is an ethnically andsocially diverse patient population across the lifespan. The Psychologistassess and evaluate individuals' problems through the use of case history,interview, and observation and provide individual or group counselingservices to assist individuals in achieving more effective personal,social, educational, and vocational development and adjustment.
Provides risk assessments of behavioral health conditions (e.g. PHQ9,SBIRT, etc.) for the purpose of advising primary care providers,referral to psychiatrist and/or other appropriate level of care.
Consults with primary care providers to improve patient functioning and healthstatus.
Provides patient care within scope of services.
Leads individual behavioral health and health promotion interventions.
Coordinates care with center and community providers to ensure delivery ofintegrated comprehensive primary care services.
Participates in continuous quality improvement activities.
Provide psychotherapeutic services to patients .
Provide assessment through a variety of psychological and/or psychosocialevaluation/testing techniques.
Provide counseling through a variety of treatment modalities, with anemphasis on brief treatment, group work and family therapy.
Act as liaison with other community agencies on the client's behalf whenappropriate.
May participate in supervision as requested by the Director of BehavioralHealth.
Provide consultation and education to other agency staff.
Provide clinical supervision to non-psychologists
Responsible for documenting service encounters in a thorough, accurate andtimely manner. Documentation shall be compliant with supervisory instructionand according to protocols established by the health center and Medicaidstandards.
Participates in periodic training, and satisfactorily completes requiredtraining in a timely manner.
Responds to emergency activity within the office according to protocol.
Complies with all policies, procedures, and standard work.
Complies with the environment of care, patient safety and infection controlprocedures.
Maintains patient confidentiality, in accordance with HIPAA guidelines.
Uphold the Mission Vision and Values of Uptown Community Health Center.
Associates will dedicate 100% of their full time equivalent to UptownCommunity Health Center.
