Position Summary
Under the direction of the HIV Program Director, the PRPM will help facilitate, coordinate, educate and bring awareness to the national efforts to end the HIV epidemic while helping to prevent the spread of HIV through testing, linking and retaining HIV+ patients in care. The PRPM will educate the current patient population working with the HIV team and mobile medical treatment unit, the Highlands Health Van (HHV), to engage new populations in a vigorous HIV prevention outreach effort. This effort will encompass two pillars of care 1.) Prevention, stopping the spread of HIV through reducing transmission, via providing PrEP (PreExposure Prophylaxis) for high risk individuals. 2.) Retention, keeping HIV + patients in care and maintaining viral load suppression (VLS) rates to undetectable. These outreach effort’s will be conducted in the six ZHC counties, however the primary location will be in the Essex County area where Zufall’s West Orange site is located. This area was indicated as a specific pocket of need for HIV prevention services. Additionally, the PRPM’s role is to help assist and support the clinical team in providing the highest quality patient care to our current and future patients. The PRPM will help direct patients and/or the appropriate community agencies to assist with testing, linkage to HIV care, outreach, and/or retention in HIV care for anyone diagnosed/newly diagnosed with HIV. The PRPM will increase awareness to PrEP in high-risk individuals and expand the Zufall medical providers prescribing PrEP and linking to care for PrEP seeking individuals.
Essential Functions, Duties, and Responsibilities
The PRPM will engage new populations in a robust HIV prevention outreach effort, conduct prevention education, identify and secure resources for all patients regardless of insurance status, and serve as or identify staff that will serve as a PrEP champion/navigator(s), increasing expertise among providers to enhance system-wide capacity to provide PrEP education. The manager will enhance Provider education for uniform delivery of technically competent, culturally competent and stigma-free care. The Manager will supervise all the ending the epidemic (ETE) health workers, increase in/outreach including partner testing’ and provide PrEP referrals and linkage to care through augmented in-reach to high-risk patients testing negative for HIV.
Develop and evaluate protocols, procedures, and educational materials for use in clinics, consistent with state guidelines and departmental policies for PrEP
Coordinate data collection, input, analysis, and reporting of program outcomes, and complete monthly, quarterly, bi-annual, and annual reports, as per funder or department requirements
Provide supervision for PrEP Champion(s) and ETE staff
Coordinate with the HIV Program Director and clinic staff to establish, monitor, and evaluate clinic and van HIV testing services, education, linkage to care, and effective delivery of PrEP/PEP resources to meet standards of care
Serve as program contact for external agencies and groups regarding PrEP education, counseling, prevention, and disseminate information as appropriate
Provide PrEP counseling, coordination of care, testing services, and treatment adherence to aid in ending the HIV epidemic
Provide testing and linkage to care for any newly diagnosed or reengagements into care
Collaborate with Zufall staff (including Outreach and the mobile site manager) to coordinate and conduct prescreening for high-risk populations, education and resources on what PrEP is and who qualifies for it, continued HIV testing and adherence to the PrEP requirements and facilitate community education
Provide consultation and referrals of individual with concerns about high-risk behaviors and preventing the spread of HIV infection. Work with management on related policy development
Provide information to other city departments, community agencies, and health professionals as directed or required
Collaborate with the community agencies to support the goals and objectives for the PrEP ending the epidemic prevention, care, and treatment programming
Assess and identify gaps in program delivery and work with Program Director and staff to resolve barriers/stigmas around PrEP
Create new MOU’s/MOA’s with external agencies to work on prevention and retention of current and future patients
Monitor care logs and patient visits to retain in care and comply with the PrEP guidelines
Capacity building and development with community organizations
NEMA/TGA planning council
Help coordinate and co-manager the CAB meetings with consumers
Performs other duties as assigned by HIV Program Director/Chief Medical Officer
Knowledge, Skills and Abilities
Strong analytical, problem-solving, and assessment skills
Excellent written and verbal communication skills, including public speaking, counseling, and education
Excellent interpersonal skills and ability to work cohesively with a team
Excellent time management skills
Highly organized and able to meet multiple deadlines
Knowledge of Word and Excel required
Experienced in the use of electronic medical records
Self-starter and the ability to work independently with minimal supervision
Learns and applies trauma informed principles within the scope of the position
Education and Experience
Bachelor’s (Master’s Preferred) degree in public health, social work, health services, or related field, alternatively proven expertise and experience delivering HIV prevention, care, and treatment service delivery, preferably within a public health/hospital/clinic-based setting
Knowledge of HIV, in prevention and retention services
Familiarity with HIV/AIDS, substance use, and mental health
Ability to work cooperatively as a part of a team in a clinical and non-clinical setting
Supervisory experience of a minimum of 2 years
Bilingual (Spanish)/bicultural preferred
Excellent oral and written communication, organization, and computer skills
