Compensation (Pay): $117,374.40 to $144,355.71/annually, based on experience. *Other compensation may include signing bonuses, rural differentials, and relocation reimbursement. Benefits:

Peak Vista Community Health Centers is seeking a full time, experienced advance practice provider to join their team of providers. This position is specific to providing AWVs to Medicare Beneficiaries. This position will travel to health centers with a high volume of Medicare patients, as well as do in home AWVs. The position offers great quality of work/life balance with either four 10-hour days or five 8-hour days, no in-patient duties, no on-going chronic condition management, no assigned panel of patients, and occasional telephone call backup to the after-hours triage service. This is a 12-month pilot project. Future employment will be determined upon completion of project.
Essential Duties and Responsibilities include the following:
Complete AWVS for Medicare beneficiaries
Complete “Welcome to Medicare” visits for Medicare beneficiaries
Accurate review of medical history
Medication review, reconciliation and education, medication adherence and storage/disposal plan
Assessment of height/weight, blood pressure, hearing and other routine measurements of wellbeing such as Body Mass Index
Detection of cognitive impairment using accepted tools of assessment (mini-mental status exam)
Evaluation of personal/home safety and Fall Risk Assessment using accepted tools (TUG, Tinetti Balance Assessment)
Screening for Anxiety, Depression, and alcohol/drug misuse using accepted tools of assessment (GAD-2, GAD-9, PHQ-2, PHQ-9, GDS, CAGE, AUDIT)
Incorporation of self-determination and advanced planning into the care plan. Discussion, education, identification of end-of-life goals, and/or development of a treatment plan, using accepted/legal tools and resources (Five Wishes, Conversation Project, Colorado Advance Directives Consortium, CHA Your Right to Make Healthcare Decisions).
Development of a plan for wellness and preventive services
Identify patients for referral to additional services (BH, diabetic education, etc.)
Implements interventions appropriate for identified patient needs
Identification of Patients for Chronic Case Management
Accurate patient information review, evaluation, communication and documentation
Advises patients in lifestyle changes to optimize personal health outcomes. Assists patients in managing chronic health conditions.
Maintains accurate records of patient's medical history, prescribed medications, allergies, diagnoses, and progress.
Communicate patient plans with Primary Care Providers
Accurately codes for services provided
Demonstrates understanding in coding/billing for AWVs
Ensures services and conditions are accurately coded for billing/reimbursement
Recaptures all appropriate HCC codes for patients
Work with support team members to ensure AWVs are scheduled
Work closely with the VP of Population Health to ensure PVCHC has a viable approach to ensuring Medicare beneficiaries receive AWVs
Assists in providing training regarding AWVs to the organization.
Demonstrates understanding of Health Related Social Needs
Maintain open line of communication between the Health Informatics department and all other internal departments and patients as necessary
Exhibits positive leadership skills, actively striving to improve the patient and staff experience.
Demonstrates knowledge of and adherence to the Compliance Plan and conflict of interest requirements. Enhances the effectiveness; efficiency and productivity of the department by contributing to, and participating in, departmental performance improvement goals
Ability to frequently travel to all Peak Vista Community Health Center sites on a regular basis in personal vehicle
Self-directed project management and strong time management skills

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Perform all job functions and responsibilities of a quality assurance specialist and provide team education and second level review. Preferred 2+ years of geriatric experience. Case management experience preferred.
Education/Experience: Our ideal candidate has 2+ years of experience in family medicine practicing in a primary care setting; AGPCNP preferred, able to utilize a template based EMR (NextGen); has an unrestricted Colorado license, as well as an active DEA, and is comfortable seeing all ages, and averages 12 patients per 10 hour day. Must have a strong desire to make a difference in the community. Must embrace a team-oriented approach.
Language Ability: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Math Ability: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills: To perform this job successfully, an individual should have knowledge of internet, Microsoft Office (Word, Excel, Outlook, Power Point), other relevant software programs.
Certificates and Licenses: Current CPR (BLS), Medical License, Current CO Driver’s License
