Job Summary
The Physical Therapist ("Therapist") under minimal supervision,assesses, plans, organizes, and participates in rehabilitativetreatments that improve mobility, relieve pain, increase strength, andimprove or correct disabling conditions resulting from disease or injury.
The Therapist coordinates care of patients in collaboration with healthcareprofessionals, family and caregiver. Utilizes experience and education toassist with developing programs, improving performance, and studentandworkforce education.
The Therapist is a subject matter expert that relies on clinical reasoning insynthesizing complex evaluations and routinely collaborates with colleaguesand key stakeholders to coordinate patient treatment that issafe, efficient and high-quality.
Patient care visits are 1:1 in private rooms.Visits are 45 minutes in length to give the therapist focused time to theirpatients.Mentorship program is provided to all new employees.
Essential Functions:
Patient CareDeliver, assess, plan, organize, and participate in rehabilitativetreatments that improve mobility, relieve pain, increase strength, andimprove or correct disabling conditions resulting from disease or injuryutilizing evidence-based occupational therapy treatment interventions. Performand document an initial exam, evaluating data to identify problems anddetermine a diagnosis prior to intervention. (30%)Treatment Plan (Plan of Care)Develop and maintain the patient's plan of care in collaboration withPhysician's, the patient and family. Focus the plan of care on thepatient's age and lifestyle to determine measurable long- and short-termgoals and a defined D/C plan. Discuss the proposed intervention, itsmaterial risks and expected benefits, and any reasonable alternatives withpatient and family members. Modifies treatment plan to coincide withpatient's goals and the overall plan. Confer with the patient, medicalpractitioners, or appropriate others to plan, implement, or assess theintervention program. (30%)EvaluationEvaluate the effects of treatment at various stages and adjust treatments toachieve maximum benefit. Evaluates patient outcomes based on presentingdiagnosis and pertinent information regarding the patient. (15%)DocumentationComplete patient care documentation recording the prognosis, treatment,response, and progress in patient's chart. Update medical recordsaccording to prescribed departmental standards. Complete appropriate databases, problem lists, treatment goals and individualized plan of care.Completes patient charges per department guidelines. (10%)Education (Patient and Employees)Assists in precepting, orienting and teaching students (OT students,hospital staff), precepts new employees during orientation. Trains andeducates patients, family members and care-givers in therapy principles,methods and techniques in order to achieve intended outcomes. (5%)DischargeDischarge patient from physical therapy when goals or projected outcomes havebeen attained and provide for appropriate follow-up care or referrals. (5%)Staff SupervisionDirect, supervise, assess, and communicate with supportive personnel.Assist in overseeing the activities of the staff which includes but is notlimited to sourcing, hiring, and training new team members, creating andmanaging schedules, evaluating performance, and providing coaching toengage and motivate team members, identifying and applying careeradvancement opportunities for team members, and helping to resolve employeeiss es and disputes. (5%)
Education:
Master's Degree Required
Work Experience:
1-3 years Previous experience in acute care environment Required
Licenses:
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required
Knowledge, Skills and Abilities:
