　　Location: Memorial Hospital Central, Colorado Springs, CO

　　Department: Pharmacy Residency Program

　　FTE: Full time, 1.0, 80 hours per pay period (2 weeks)

　　Shift: Rotating

　　Pay: $30.17/hr

　　Requirements:

　　Minimum Required Education: Doctor of Pharmacy from an accredited school of pharmacy.

　　Required Licensure/Certification: State licensure as a Pharmacist within 120 days of hire.

　　Responsibilities:

　　Manages pharmaceutical care and protocols as a part of an interdisciplinary team.

　　Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.

　　Provides drug information and education to medical staff and patients.

　　Documents interventions and their outcomes for use in drug therapy quality improvement efforts.

　　Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
