DUTIES AND RESPONSIBILITIES include the following.ESSENTIAL DUTIES ARE:Provides non-medical personal care as assigned,Furnishing services in accordance with agency policies,Provides accurate and timely documentation of care in accordance withagency policy and pay source requirements.Participates, as requested, in orientation of new staff, in-serviceeducation, continuing education, utilization review and QI/QA programs.Uses agency approved software to enter evaluations and visit documentation.
OTHER DUTIES: Other duties as assigned.
SUPERVISORY RESPONSIBILITIES:This job has no supervisory responsibilities.
EDUCATION and/or EXPERIENCE:Experience of a minimum of six months as a personal care giver preferred,skills validation required.
ABILITIES, SKILLS AND KNOWLEDGE:ž Ability to pass a skills validation testž Ability to read and interpret physicians' orders and notes fromother caregivers as well as respond to them verbally and in writing.ž Ability to effectively present information to clients andemployees and community.ž Ability to respond effectively to the most sensitive inquiries orcomplaints.ž Ability to define problems, collect data, establish facts,and draw valid conclusions.ž Ability to establish and maintain effective working relationshipswith fellow employees, the public, and supervisors.
SPECIAL REQUIREMENTS:ž TB screening upon hire and annually thereafter;ž Possession of reliable transportation; if driving personalvehicle a valid Driver's and applicable car insurance.ž Proof of current Covid-19 vaccination or approved Medical orReligious exemption.
PHYSICAL REQUIREMENTS:ž Ability to regularly sit, stand, stoop, bend, kneel,walk, balance, crouch, crawl, push, pull, reach with hands andarms and talk anž Ability to frequently use hands to finger, handle, or feel.ž Ability to occasionally climb and use sense of smell.ž Ability to frequently lift and/or move up to 50 pounds andoccasionally lift and/or move 100 pounds.ž Ability to perform the anticipated level of physical assistrequired to provide safe client care. If the anticipated level of assist isgreater than that which the employee can safely provide, the supervisor isto be notified immediately.ž Ability to use close vision, distance vision, color vision,peripheral vision, depth perception.ž Ability to adjust focus.
WORK ENVIRONMENT:ž The employee is frequently exposed to outside weather conditions.ž The work environment is highly variable, as care is provided inprivate homes.ž Lighting, acoustics, air quality, temperature andenvironmental hazards are not predictable.ž The employee is occasionally exposed to wet and/or humidconditions, household pets/animals, fumes or airborne particles, andtoxic or caustic chemicals.ž The noise level in the office work environment is usually moderate.
TRAVEL:The employee is required to travel frequently to complete home health visits.
