Must have a clear APS/CPS check & clear criminal background. 28-32 hours a week, Mon-Thurs. Provide services to Waiver recipients on an individual basis or ground basis in a work activity center licensed by OHFLAC or and integrated community or natural setting. Person-Centered Support services consist of individually tailored training and/or support activities that enable the Member to live and inclusively participate in the community in which the member resides, works, receives their education, accesses healthcare or engages in social and recreational activities. The activities and environments are deigned to increase the acquisition of skills and appropriate behavior that are necessary for the member to have greater independence, personal choice and allow for maximum inclusion into their community. Essential Duties: Must have the ability to perform all tasks as defined by the Service Coordinator. May be used to assist with the acquisition, retention, and/or improvement of the following areas of functionality: 1. Community Involvement, 2. Personal Hygiene, 3. Meal Preparation, 4. Health Education, 5. Communication, 6. Problem-Solving, 7. Social Skills Development and Coping Skills, 8. Social Competence. Must be available to work weekends, holidays, and occasional overtime if needed. Must have the ability to deal with abrupt schedule changes. Ability to communicate with individuals that are physically/mentally challenged. Answering the telephone, taking concise messages, and providing clear and correct information. Perform any and all duties incidental to the position. Must be eighteen years or older. Must have a valid driver's license, clear criminal background and adult protective services check. Must have dependable transportation with minimum liability vehicle insurance. Must meet all qualifications and training requirements including, but not limited to: CPR certification, first aid certification, blood borne pathogens, CPI (non-violent crisis intervention), policies and procedures, training in Member specific issues, HIPPA, recognition and reporting abuse and neglect, documentation requirements, regulatory requirements, and passanger services and safety.
