  Description:

  The patient safety and quality improvement specialist provides leadership, consultation and facilitation of the design, development and implementation of an effective patient safety and quality improvement program that evaluates, improves and measures patient safety and quality of care/service for the assigned areas of accountability. The patient safety and quality improvement specialist provides the tools, techniques and skills necessary for patient safety, outcomes measurement, process improvement as well as thorough and credible root cause analysis processes

  Education Requirements :

  Bachelor's degree in Nursing preferred; Associates degree in nursing required

  Experience Requirements :

  Two years of demonstrable work experience in patient safety, use and management of databases, trend analysis, quality improvement or comparable experience preferred; strong organizational skills; strong communication and facilitation skills. Basic computer skills are required; use of computerized data management systems, and statistical control tools and techniques preferred.

  Responsibilities:

  Provides leadership to the clinical and leadership staff in the design, development, implementation and evaluation of an effective multidisciplinary patient safety and quality program for areas of accountability that is consistent with and supportive of the hospital-wide patient safety and quality improvement system and initiatives.

  This patient safety and quality improvement program demonstrates continual improvement in patient and process outcomes measured by clinical, cost, access/service and satisfaction parameters and integrates interdisciplinary processes that fosters team collaboration while maintaining an effective professional care review process.

  Contributes to a culture of inclusion & leveraging diversity that unleashes talent, creativity & greater engagement in the patient safety &/or quality improvement activity

  Facilitates and supports review and analysis of outcome data to identify practitioner or system variation and develop strategies to effectively reduce variation.

  Leads an interdisciplinary team that successfully achieves measurable improvements in quality of care/service.

  Facilitates the development of valid, reliable quality indicators; the achievement and integration of measurable improvements in outcomes that reflect all aspects of care and service by the parameters of: clinical, cost, satisfaction, access/service; regulatory (including but not limited to payor, internal and community needs, DOH, JCAHO, SMDA).

  Establishment and maintenance of database required to abstract data and/or monitor indicators identified for outcome and process improvements and that profiles the assigned program, level or department of care including but not limited to: measures of performance for internal and external databases (e.g. Midas, PCIRS, ACC, STS, NYSCSRS, NYSCARS, DOH Perinatal, NSQIP): outcomes that measure clinical/service, cost, access, satisfaction, health status; regulatory; significant/sentinel events; practitioner specific profile.

  Identifies appropriate study and methods for data collection and analysis.

  Uses the outcome data with all disciplines to focus improvement activities.

  Actively pursues comparative outcome database and benchmarking opportunities and makes recommendations for participation.

  Identifies trends and opportunities for improvement and facilitates those improvements through an interdisciplinary process

  Identifies barriers to achieving improvement and takes effective action to address.

  Identifies during investigations and analysis of events and/or outcomes processes that need to be considered for redesign.

  Facilitates the development of effective plan that achieves the process redesign needed.

  Directs and guides the implementation of the improvement plan.

  Ensures that the process redesign is evaluated for effectiveness.

  Develop and maintains current knowledge of and facilitates compliance with NYS Hospital Code (405, NYPORTS), IPRO, Joint Commission standards, CMS, SMDA and other recommendations, regulations or requirements for patient safety and quality measures, indicators and monitoring activities.

  Has an up-to-date working knowledge of pertinent issues relative to Patient Safety, external demands for outcomes data and from payors and what is necessary to both defend and prove quality of care and service in their assigned program, level or department of care.

  Serves as a liaison with the DOH and other regulatory agencies during onsite investigations; collaborates with others in the investigation and successful resolution of regulatory deficiencies and/or opportunities for improvement

  Develops and maintains an ongoing process for timely, accurate identification of events and/or significant/sentinel events for assigned program, level or department of care that is effective in identifying trends by program, practitioners and across all patient populations.

  For those events that meet SPH definition of significant/sentinel, completes timely, accurate fact finding/investigations, preliminary assessment of risks (to other patients, regulatory, satisfaction, claim/litigation, media) and works with accountable leadership to initiate appropriate level of review.

  Presents preliminary assessment of risks at weekly Patient Safety and Quality/Risk Management intradepartmental events meeting to validate assessments and to ensure appropriate follow-ups includes all relevant persons.

  Facilitates effective interdisciplinary review and/or root cause analysis of events that identifies opportunities for improvement, develops measurable action plan to prevent reoccurrence, ensures development of and/or evaluation of action plan as well as identifies and addresses trends in practitioner and/or system/process variation.

  Pay range: $31.50-$45.68

  Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
