　　DescriptionPosition Summary/Essential Duties:The PCNA provides personalized care to patients in support of their healing, wellness and overall experience as delegated by the RN/LPN. Duties include:* Assists patient with daily hygiene and grooming as needed* Assists patients with their daily meals as needed* Ensures patients are out of bed and in the chair for meals.* Provides oral care for patient specific needs* Ensures linens are changed and clean* Assists patients with basic necessities (pantry items and other hospitality needs)* Assists as needed with patient mobility, repositioning needs, and other activities of daily living* Assist with obtaining vital signs as needed* Documents in the electronic medical recordEqual Opportunity Employer -- minorities/females/veterans/individuals with disabilities/sexual orientation/gender identity
