General Summary:
The Palliative Care Social Worker works in collaboration with the other members of the Palliative Care Program to provide patients and their families with palliative care services including goals of care planning and symptom management. The position supports the Mission of Trinity Health and Holy Cross Hospital.
Minimum licensure/certification required (if applicable):
State of Maryland LGSW, LCSW or LCSW-C licensure, LCSW-C preferred.
Certification in Palliative Care Social Work preferred.
Minimum Knowledge, Skills, & Abilities Required:
Master's Degree in Social Work.
At least one year of social work experience in a health or mental health care setting. Demonstrated ability to work with all age groups of patients. Able to communicate effectively, demonstrating good audio and visual acuity. Ability to prioritize tasks and manage time effectively. Computer skills in Word, Excel, and ability to learn hospital EMR and other electronic tools.
