Avivo's Ending Homelessness division provides pathways to housing for individuals experiencing homelessness. We work with county coordinated entry systems and tribal governments, as well as operating a unique street outreach teams to connect unsheltered individuals with shelter and housing. We operate Avivo Village to address the needs of individuals experiencing unsheltered homelessness. Avivo Village is a low barrier and culturally responsive shelter with individual "homes" focused on moving residents to permanent housing. Avivo's Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma informed service delivery. The Street Outreach Navigator will work specifically, but not exclusively with individuals who are experiencing long-term, chronic unsheltered homelessness, and have an opioid use disorder. The Street Outreach Navigator will be responsible for helping individuals obtain housing, referrals to appropriate resources, assist with access to basic needs, support individuals with substance use or mental health treatment goals, and establish medical care through progressive engagement. The Street Outreach Navigator will be community based, supporting people in their current environment which may include streets, under bridges, in parks, camping, and other locations unfit for human habitation. The ideal candidate for this role thrives in an unpredictable work environment and is willing to work partially outside year-round. Our Ending Homelessness teams utilize Housing First and Harm Reduction models. PRIMARY RESPONSIBILITIES Provide housing-focused street outreach services Provide Outreach services to those experiencing literal homelessness and those unable to be inside during daytime hours, connecting them to supportive services, housing options, and ensuring their basic needs are met. Coordinate with Avivo Supportive Housing programs, other service providers, and the Coordinated Entry System to help clients achieve stability and work towards housing. Accurately complete paperwork in a timely manner. Including, case notes, data collection, and monthly reporting for assigned clients utilizing online databases including HMIS and Credible. Diligently protect client's data according to HIPAA standards. Ensure services provided are culturally responsive utilizing best practices. Complete client documentation to meet timeline and program standards including applicable state and certification standards. Qualifications High school diploma required. BA in Behavioral Science preferred or Lived Experience of homelessness and/or SUD/Mental Health recovery. One or more years of experience working with single adults experiencing homelessness or similar experience. Experience with and/or a demonstrated commitment to developing and delivering culturally responsive services. Must have access to a personal vehicle. Mileage reimbursement will be provided. Must have a valid, current driver's license, proof of valid insurance and a clean driving record. PREFERRED: Lived experience of homelessness and/or SUD recovery. Street outreach experience working with unsheltered individuals. Experience with programs serving individuals with dual diagnoses. Experience using HMIS and/or Credible. Experience working in Harm Reduction and Housing First with a demonstrated working knowledge and commitment to these modalities.
