Position Purpose/Summary Responsible, full-time outreach work performedunder the direct supervision of the Director of Elder Affairs for adiverse population. Responsible for providing case management servicesand application assistance to Springfield seniors aged 60 and older, andtheir caregivers. Facilitates access to appropriate resources andservices, including but not limited to: Medicare, Medicaid, fuelassistance, food stamps, housing, Social Security, transportation, legalservices, financial planning, adult day health programs, and nursinghomes. Work is performed under the supervision of the Director of ElderAffairs. Supervision is exercised over other employees. EssentialFunctions Provides all program-specific reports to the Director forreview and submission as needed. Works to develop relationships withelders, staff, city departments and community agencies in an effectiveand meaningful manner. Coordinates the summer Farm Share Program, BrownBag, Farmers\' Market Coupon Program, with community agencies.Coordinates support group meetings for those in need who have lost lovedones, have memory issues, changes in financial status, etc. Documentsidentified needs of clients and maintains up-to-date and confidentialfiles. Enters data into \"Myseniorcenter\" database, preparesdaily/monthly statistics and status reports for all clients. Developscare plans, makes appropriate referrals and provides basic caremanagement. Files must include intake, assessment form, progress notes,and a signed client authorization form. Acquiresapplications/forms/instructions for public/private programs, explainsentitlement and benefit programs, and explains all procedures, appealsand eligibility requirements. Assists seniors in the filling out of allappropriate and necessary applications and forms. Maintains copies ofcurrent and up-to-date applications, resource materials and handouts anddistributes them in appropriate locations. Conduct home visits withclients to assess home safety, investigate claims, and assist withtransportation. Advocate for seniors\' rights, entitlements, etc. Mayattend court cases to advocate for seniors when no other family memberis present. Assists seniors and caregivers in providing for anenvironment to reduce social isolation, financial hardship, and promoteways to maintain their independence and involvement in the community.Plan, develop and implement a wide variety of educational forums forelders with regard to social, emotional and health awareness. Performother related duties as required. Knowledge, Skills, and AbilitiesAbility to express oneself clearly and concisely both orally and inwriting. Extensive knowledge of senior programs, resources, and servicesAbility to maintain flexibility when scheduling, making home visits andoverall travel plans. Must be able to relate well with older citizensand the community members. Extensive knowledge of social, cultural andrecreational needs of elders. Strong interpersonal skills to handlesensitive and confidential information. Ability to be a team leader forthe DEA outreach team. Knowledge of federal, state and local servicesavailable to older adults. Must be able to develop comprehensivecurriculums that meet the needs of elders Education and ExperienceBachelors Degree in Social work or related field. Master?s degreepreferred. Licensed social worker preferred. Minimum of three (3) yearsexperience working with seniors. Bilingual preferred.
