  Your Specific Responsibilities:

  This Registered Nurse position is responsible for providing comprehensive nursing services to women and their families via the Healthy Start Program. This position promotes maternal and child health in Hendry and Glades Counties. Position reports directly to the Social Services Counselor – SES and is headquartered in Clewiston.

  Implements the Healthy Start program for families referred to and enrolled in Healthy Start in a manner consistent with the Healthy Start Standards and Guidelines and the Healthy Start Coalition contract. Provides the Healthy Start services of care coordination, initial assessments, parenting support/education, childbirth support, smoking cessation, breastfeeding support, and outreach. Assists families in assuring access to prenatal or infant care and community resources. Assists clients with forms. Provides or arranges for social, medical, or educational services. Provides services in a location that best meet the needs of the family through home visits. Monitors Healthy Start participants’ access to prenatal and infant services. Responsible for the development of Family Support Plans.

  Reviews FSW charts on a bi-weekly basis. Participates in Quality Assurance/Quality Improvement activities. Maintains and updates client files as required for Healthy Start using the Well Family System, HMS, and clinical records. Documents service delivery and monthly reports as required for Healthy Start.

  Acts as a liaison with community agencies and service organizations through referral and transitional services. Attends scheduled meetings, training sessions, and outreach events. Promotes public awareness of the Healthy Start Program and develops and maintains community relationships to support client referrals.

  Required Knowledge, Skills, and Abilities:

  Knowledge of nursing and public health principles, practices, and techniques related to maternal child health. Must possess a valid driver's license and have the ability to travel between multiple work sites and flex work schedule. Must possess basic word processing skills. Ability to participate in the planning and implementing of treatment plans for clients. Ability to work independently and to plan, organize and coordinate work assignments. Ability to communicate effectively. Ability to establish and maintain effective working relationships with others. Ability to effectively interview and assess the health care of clients.

  Qualifications:

  Minimum:

  Willing to work during or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent of this position includes working in Special Needs or Red Cross Shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, manmade or natural

  Have an Associate's Degree and Licensure as a Registered Professional Nurse, plus one year of experience working with children or families in a social service, health/medical, nursing, child development, or childcare setting

  Willing to work a flexible schedule (evenings/weekends as needed) to meet the needs of clients served
