  400 W. Airport Blvd Sanford, FL 32773This is highly confidential and specialized position dedicated to providing nursing services for persons living with HIV/AIDS. This dedicated care position is part of a team that are providing enhanced care coordination services to persons living with HIV in Seminole County. Increase the numbers of persons living with HIV/AIDS who engage and remain engaged in HIV primary-specialty Care, increase the number of persons who obtain insurance and increase the number of persons maintain viral load suppression.

  Apply nursing principles and techniques in the care and treatment of clients, administer prescribed medications ensuring the Rights of Medication Administration, be sensitive when interviewing clients about a sensitive communicable disease issue such as HIV/AIDS or sexually transmitted disease, utilize nursing protocols in the care of clients, provide client and family education and make appropriate referrals as indicated, evaluates immunization status and administers immunizations per protocol using correct techniques, log administered vaccines into Florida Shots, Health Management System (HMS) and Group Ware – Provide Enterprise System, enter medication refill requests into HMS – Emdeon, Florida Ryan White Portal and place on hold for provider to review and authorize, accurately documents all services provided.

  Assist with chart reviews and other QA/QI activities to improve clinic functions and to meet DOH-Seminole’s mission statement and goals. Review lab results, consult with providers and provide follow up to those assigned as abnormal or priority especially viral loads and CD4 counts. Assess client’s well-being including but not limited: medication adherence, reasons for missed appointments, housing conditions, transportation., provide and implement clinical recommendations. Provide care coordination especially to newly diagnosed people living with HIV/AIDS. Serve as liaison with team of State Telehealth Providers.

  Conduct outreach activities for potential clients to educate and promote the availability of services. Incumbent will participate in health fairs, community events and collaborate with providers among other activities.

  Enter all client encounters – both face to face and by phone into the Health Management System (HMS) and Provide Enterprise System for tracking and reporting purposes. Submits required reports, maintains assigned records, completes/returns all paperwork pertaining to job duties accurately, legibly and in proper order. This would also include timely submittal of timesheets, mileage, and EARS.

  Actively participate in team meetings or case conferences for the clients to improve assessment and to sustain retention in care. Participate on case discussions and staffing.

  Maintain appropriate relationships with Key Point of Entry (KPOE) into the healthcare system.

  This position may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disasters or threat of disaster, man-made or natural. This position will maintain confidentiality and security of records as specified by departmental regulations and laws.

  Knowledge, skills and abilities, including utilization of equipment, required for the position: Must be bilingual (English/Spanish preferred), terminology, principles and analytical techniques of interviewing; data collection and analysis principles and procedures. Ability to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to Ryan White, ADAP, State and Local Specialty Care HIV/AIDS Programs, understand and follow legislative policy direction as established in Federal and Florida Statutes; deal with the public in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others; utilize problem solving techniques and use personal computers.

  Qualifications:

  Valid state of Florida Registered Nursing License that is in good standing (with no complaints filed through the Florida Board of Nursing)

  Verifiable experience working with any of the following programs: Immunizations, Family Planning, STD, HIV/AIDS, School Health, TB/Refugee Health

  Florida Department of Health is required to work before, during, and/or beyond normal work hours or days in the event of an

  emergency. Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties

  including but not limited to, response to or threats involving any disaster or threat of disaster man-made or natural.
