　　SUMMARY

　　Clinical Dietitian using the Nutrition Care Process, and provides interventions for long-term care residents of acute nursing facility using the electronic medical record system (EPIC).

　　STATUS: Per Diem

　　LOCATION: Multiple: St. Mary's Campus, Edna Tina Wilson Living Center, & Park Ridge Living Center

　　DEPARTMENT: Food and Nutrition

　　SCHEDULE: Day, partial remote option

　　ATTRIBUTES

　　Bachelor's Degree in Nutrition or related field

　　Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) within six 6 months of hire

　　One (1) year experience in a Healthcare facility

　　Strong analytical, computer, and communication skills

　　RESPONSIBILITIES

　　Conducts comprehensive nutrition assessments using evidence-based practices

　　Manages nutrition support for patients on vented unit and residents in long-term care

　　Administers patient/family nutrition education and counseling

　　Treats/prevents malnutrition, chronic wounds, and complications related to nutrition supporting patients with multiple comorbidities

　　Participates in care planning meetings as necessary, and collaborates with members of the multidisciplinary team to provide Medical Nutrition Therapy

　　Meets standards for DOH/state/federal and facility

　　EDUCATION:

　　BS: NutritionPHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

　　Registered Dietitian - Commission on Dietetic Registration (CDR)PAY RANGE: $26.00 - $33.66

　　The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
