Organization Overview AdLib, Inc. is a non-profit that focuses onempowering people with disabilities to access and manage benefits,services, and resources to live independently in the community. We are aconsumer controlled, consumer directed service that assists people withdisabilities to achieve a greater level of independence. As a Center forIndependent Living we provide the five core services: information andreferral, skills training, advocacy (systemic and individual), peercounseling, and transition (from nursing home to community and youth toadulthood). In addition to the core independent living services weoperate the MassHealth (Medicaid) funded personal care managementprogram, SSA Representative Payee program, MRC vocational rehabilitationservices, and long term care options counseling. Nursing Home Transition AdvocateThe Nursing Home Transition Specialist will identify and assist peoplewith disabilities to move from institutions to community based settings.The position involves extensive contact with nursing home professionals,consumers, family members, and other human service providers. Workincludes teaching consumers how to access benefits and services such ashealth insurance, housing, social security, and food stamps. Thisposition will need to be able to learn and utilize MassHealth waiverprograms specifically designed to help people transition out ofinstitutions. In addition, the NHT Advocate will provide mentoring,skills training, and information & referral services. EssentialFunctions: Work closely with other staff, nursing home professionals,and state designated staff to determine consumer\'s eligibility forvarious programs; regularly interact with consumers, their families, andcaregivers; conduct and document assessments for services; understandand be able to explain how to apply for utilize MA benefits; enteringrequired documentation correctly and timely in both electronic andwritten formats; assist consumers in problem solving so that they areable to housed in the community. This position is full-time 40 hours (1hour paid lunch daily) and 75% of work takes place in the Pittsfieldoffice or in the community/nursing homes. This position requires travelwith personal vehicle, home visits (ascending/descending stairs),electronic and written documentation, and ability to understand benefitrules and regulations. Primary working hours are 9am-5pm M-F.
