Nurse Practitioner for DMC Home Visit Program

　　Join a great team! DMC Primary Care, a physician-owned, independent practice, is seeking a full-time experienced Nurse Practitioner to join our growing team of primary care providers.

　　Responsibilities:

　　The successful candidate will work closely and collaboratively with providers and our care management team including performing comprehensive in-home assessments for identified patients, transitional care post discharge, and assisting the primary care team in chronic disease management. Our goal is to provide an enhanced type of primary care that offers high quality, expanded services for our patients with chronic conditions, and personalized patient education.

　　Position Highlights:

　　Offers flexible schedule – working either 5 days or 4 days per week with an extended day

　　Travel primarily in the Rockingham, Hillsborough and Merrimack counties with mileage reimbursement

　　Paid time off, comprehensive insurance benefits, and 401K

　　Qualifications:

　　State of New Hampshire Nurse Practitioner license

　　Nurse Practitioner Certification from either the ANCC/AANP

　　3+ years of clinical experience as a FNP in the primary care or home care setting

　　Working knowledge of chronic care management and various medication options

　　Current DEA licensure

　　Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care or home care

　　Previous nursing experience is a plus
