Opportunity for a Nurse Practitioner to provide value-based care toBone Marrow TransplantandGeneral Oncologypatients in primarilyoutpatientsettings, located atHartford Hospital, and surrounding sites in Hartford County. Some highlights of this opportunity include: Bone Marrow Transplant Nurse Practitioners (APRN) function in a split role between bone marrow transplant and general hematology oncology. * Bone Marrow Transplant responsibilities include: o Managing bone marrow transplant patients in coordination with multidisciplinary teams, including pre-bone marrow transplant evaluations, post-transplant follow-up and monitoring for early and late complications, as well as survivorship evaluations and immunization counseling. * Oncology / Hematology responsibilities include: o Triage and the spectrum of outpatient management: urgent visits, chemotherapy pre-treatment visits, clinical trials management, follow up appointments, and ongoing survivorship care.
