Connected Care Hub at UHN and the Integrated Care ProgramThe Connected Care Hub focuses on delivering a virtual and in-person integrated care experience for patients and their caregivers. Our goal is to reduce unnecessary hospital visits, improve patient satisfaction, and improve health outcomes by following the patient throughout the transitional period of their journey, including supporting individuals in staying at home or as close to home as possible.
During the pandemic the Connected Care Hub was established to deliver comprehensive COVID care. This clinic allows patients to be monitored at home while drawing on the expertise and capacity of home & community care supports and primary care. In addition to continued COVID and Respiratory Care for patients (including therapeutic treatments), the clinic is now being expanded to provide transitional NP support to support primary care and home and community care providers, including for patients enrolled in the UHN Integrated Care Program.
To inspire, invent and deliver tomorrow’s care the Integrated Care Program looks beyond our walls and identifies and integrates the health and well-being services and supports to provide comprehensive care to our patients. The program leverages an integrated delivery model, care team, health record and funding model to optimize patient care outcomes, as well as optimize efficiency and costs for providers. The integrated care team supports patient care and seamless transitions from community to hospital and back to community, including ongoing community management.
POSITION SUMMARY:Dual reporting to the Manager of UHN’s Connected Care Clinic and the Director UHN@Home, the Nurse Practitioner, as a member of the Program, provides patient-centered navigation, intensive case management and coordination of services to a selected group of patients as they access care within University Health Network and with partner organizations, as required.
The Nurse Practitioner is expected to demonstrate autonomy while assuming and having sole accountability and the authority to independently communicate a diagnosis, prescribe drugs, order a range of diagnostic tests, make referrals, sign-off Medical Treatment Orders, and provide some nursing care as needed for an outpatient client population of the Connected Care Clinic and Integrated Care Program. As a nursing leader, the Nurse Practitioner is expected to provide clinical care, research and education. Each Nurse Practitioner will be responsible for collaborative practice within a health care team. The Nurse Practitioner will also demonstrate leadership as a change agent and support advancement of nursing practice within the Connect Care Clinic and in support of the Integrated Care Program by supporting, coaching and training the program’s other nursing roles. The role will be on-site providing in-person care with some virtual work as required.
RESPONSIBILTIES INCLUDE:Acting as a liaison between patients, the UHN Connected Care Hub team, referring clinician, community providers, specialists, primary care providers, and other providers, to ensure all components of a patient’s care are available for both the patient and their care team as they transition back into the community. This role will include supporting Integrated Care patients when issues are escalated by community nurses for patients that are recently discharged from the hospital into the community.
Following patients through the various care settings to ensure continuity of care, both virtually and in-person as needed.
Acting as a resource to UHN Integrated Care Leads and home care providers and work collaboratively with an inter-professional team, family members, support network, and a variety of informal and formal service providers.
Work with UHN Integrated Care Leads and home and community care providers to provide patients access to the right services from the right provider, at the right time
Coordinating seamless care transitions through appropriate and effective exchange of knowledge and services related to respective health conditions and the continuum of services and resources to enhance patient self-management and quality of care.
Ensuring that the patient and system level objectives of the bundle of care are met.
Participating in the evaluation of integrated care delivery.
Regular monitoring of Program data and metrics, synthesizing data to make recommendations or act upon trends or patterns to inform care planning.
Works within UHN’s Safety and Quality policies and procedures.
Support the continued development of the Connected Care Hub.
QUALIFICATIONS:Completion of a recognized Masters degree program in Nursing
Current certificate of (licensure)/registration with the College of Nurses of Ontario as RN(EC)
Membership in the Nurses Association of Ontario (RNAO) organizations relevant to the role
Nurse Practitioner (NP) Adult or Primary Care certificate complete
Current Certificate of Competence with College of Nurses of Ontario
Current certification in Basic Cardiac Life Support
Three (3) to Five (5) years of nursing experience in an acute treatment centre
Experience in general internal medicine required, and primary care an asset.
Experience in nursing research methodology
Eligible for cross-appointment to an academic institution
Excellent verbal and written communication skills
Demonstrated experience and competence in case management is preferred, with awareness for cultural sensitivity and skills to inform and best plan and facilitate coordination of patient care.
Demonstrated recent comprehensive knowledge of community resources and health care delivery systems in acute, primary care and community settings is preferred.
Excellent customer service skills, with a demonstrated ability to quickly respond to patients and situations with resourcefulness, flexibility and adaptability is required.
Demonstrated ability to work both independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers is required.
Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and the ability to effectively persuade / influence behaviour of others to align with patient care objectives and desired outcomes, are required.
Excellent initiative, judgement, time management and stress management skills are required.
Computer literacy and basic proficiency is required with a demonstrated willingness to learn and adapt to new technology.
Fluency in English (verbal and written) is required
