Summary
Under the direction of the Clinical Nurse Manager of Perioperative Services, the Orthopedic Nurse Navigator has overall responsibility for flow of specific Orthopedic Surgical Patients for the Perioperative services. The Orthopedic Nurse Navigator collaborates with Perioperative leadership, Orthopedic Surgeons, Anesthesia, and other entities within and outside of the hospital to develop and implement an orthopedic clinic and program which allows the patients through seamlessly and safely. Develops high level relationships with surgeons and organizational leadership to promote effective positive clinical and operational outcomes.
Essential Duties and Responsibilities
Supports and promotes the mission and values of Covenant Health Ministry.
Renders professional nursing care to patients within the PAT department of surgical services, in support of medical care as directed by medical staff and pursuant to objectives and policies of the hospital
Assist patients in understanding their choice to have a total joint replacement, what to expect from the surgery, how to prepare. Serves as an essential link between patients and all other care providers. This includes, all necessary clearances, patient education classes and other support services.
Follows patients progress throughout the care continuum, including pre-operative appointments, inpatient admission and discharge plan, focusing on optimization of the whole person in preparation for a positive surgical experience.
Performs and documents a pre-anesthesia nursing assessment, including but not limited to, medical history, allergies, medications etc. and follows up with anesthesia and/or hospitalist as needed regarding questions or concerns related to patient care and treatment.
Compliant in achieving unit specific competencies and requirements related to a nurse navigator roles and responsibilities.
Involved in quality improvement planning and processes for the area of practice.
Communicates patient information appropriate for continuity and safe care through hand offs, and interaction with PCPs, other referring providers, surgeons, hospitalist, anesthesiology, physical therapy, nutritionists and other specialty areas related to the optimization of patients for surgery and the follow up care post-surgery

Job Requirements
Job Knowledge and Skills
Clinical experience preferred in the field of Orthopedics, Surgery or PACU.
Membership in a professional nursing organization and Certification in specialty preferred.
Education and Experience
Current New Hampshire RN license required
Successful completion from an approved School of Nursing preferred
An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
Working Conditions/Physical Demands
Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress related to high level of responsibility for quality care.
