Summary: To provide professional, clinical, and medical social work services to patients, families, patient representatives and caregivers in the Home Care Agency. Must meet per diem business requirements.

　　Education: Completion of Masters of Social Work required.

　　Licensure: Current Rhode Island licensure required. Licensure in MA required within 30 days of hire.

　　Experience: One year experience in home care and/or experience in geriatric social work in a health care setting required.

　　Skills and Abilities: The Social Worker must demonstrate competency in clinical assessment and treatment skills, electronic medical record documentation, communication via e-mail, text, cell phone and basic computer skills such as Microsoft Word and MapQuest. He/she must demonstrate a commitment to excellence in customer service and evidence-based practice as well as to the components of a highly reliable organization. Must have a thorough knowledge of services available throughout the state of RI and neighboring MA communities as well as an understanding of the opportunities and restrictions presented by various insurance plans.

　　Working Conditions, Physical Environment and/or Safety Requirements: Social worker must be able to travel to patients’ homes and is required to provide direct care. Patient homes will have a varying degree of cleanliness, may have pets within the home, and may involve multiple family members/caregivers. Standard precautions should be observed at all times. Knowledge of the basic tenets of infection control including the use of personal protective equipment, proper bag handling technique and proper hand washing technique are required.
