We are seeking a Blended Case Manager -- Mental Illness- Substance Abuse (MISA) to join our team in our Philadelphia, PA 19119 location.
Earn $24.63/hour.Position DetailsThe Blended Case Manager -- Mental Illness- Substance Abuse (MISA) will provide support services in the home or community setting to consumers who have co-occurring chronic mental illness and substance abuse conditions. These support services include but are not limited to: consumer advocacy, linkage with natural and community supports, assistance in managing the health care delivery system and supportive counseling. Service provision includes interaction with family members or other community support personnel based on the consumer's personal goal plan. Requirements
The ideal candidate will possess the following qualifications:Bachelor's Degree with major coursework in the human services fieldTwo years of human service experience.One year of case management experience working with adults with mental health and substance abuse diagnosis.Familiarity with and acceptance of a recovery-oriented approach to mental health care preferred.Must be knowledgeable of co-occurring disorders.CAC or equivalent preferred.Valid PA driver's license and vehicle.
