ESSENTIAL DUTIES AND RESPONSIBILITIES
The Medical Records Coordinator is responsible for managing all Emergency Medical Records (EMR) databases, ensuring all medical orders are quality-checked to meet State guidelines and comply with RiseBoro policies and procedures. Additionally, they ensure timely and accurate receipt of all medical orders to ensure continuity of services. Specific duties and responsibilities include:
Ensure client physician/provider credentials are appropriately added to the SAMS (EMR) database via New York State Education Department guidelines
Verify and update accurate records of the physician’s/provider’s license are checked in the EMR weeklyi.e., identify expired or revoked providers licenses, etc.
Verify all necessary provider information regularly i.e. (weekly, monthy?) National Provider Identification (NPI) number and practice address
Add the physician information into the EMR system
Ensure the accuracy of all patient information i.e. medical order forms
Track medical order forms to ensure proper receipt, signature within the appropriate timeframe, and return to provider per DOH requirements
Identify and communicate all outstanding medical order forms or form discrepancies with the Director of Patient Services i.e. name, address, license, and NPI number
Maintain communication with all relevant parties to expedite the processing of the medical order forms
Assist with clinical projects involving internal and external requests for information/audits; ensure all information complies with HIPAA rules and regulations
Scan and manually file all documents into the patient's electronic medical record (EMR)
All other related duties as assigned
COMPETENCIES
Demonstrated experience in the homecare or related field required
Bilingual English/Spanish speaker preferred
Knowledge of HCPLUS, Human Resources Administration (HRA) Long-term Web application, SANDATA/SAM, HHA Exchange, or similar home care platforms preferred
Knowledge of home care concepts, practices, and procedures within an LHCSA
Excellent interpersonal and customer service skills
Ability to troubleshoot and problem-solve issues as they arise
Excellent oral and written communication skills
Ability to effectively multi-task, follow up processes, and use independent judgment to produce quality work on time
Excellent organizational skills with attention to detail
Ability to be flexible and adaptable
Ability to maintain confidentiality at all times

Experience in Microsoft Office Suite; working knowledge of Homecare related databases preferred
Ability to exercise good judgment and maintain confidentiality, particularly with client information
EDUCATION AND EXPERIENCE
High School diploma or equivalent required, with a minimum of two (2) years of professional work experience in healthcare, homecare, or human services preferred
Associate degree with a minimum of one (1) year of professional work experience in healthcare, homecare, or human services preferred
Any combination equivalent to education and experience that provides the required knowledge and skills may be considered qualifying.
