Responsibilities The Home CareLicensed Practical Nurse is responsible for providing direct nursingcare as prescribed by the physician to acutely ill, and/or chronicallyill patients in their homes as well as recording clinical and progressnotes; and coordinating other patient care activities/services toprovide the highest quality of patient care in the geographic servicearea to which she/he is assigned. She/he works under the directsupervision of the Registered Nurse and/or designated supervisor.Nursing activities are limited to these in compliance with the StateLPN/LVN Practice Act, any applicable licensure/certificationrequirements and agency policies and procedures. Specific JobDuties/Responsibilities Provides high quality clinical services withinthe scope of practice, as defined by the state laws governing thepractice of nursing, in accordance with the plan of care, and incoordination with other members of the health care team, following allapplicable infection control procedures. Documents patient visitsthoroughly and completely per regulatory and payer requirements in theelectronic medical record utilizing the agency's electronic visitverification program (as applicable). Actively participates in theagency's care coordination process including timely reportingpatients' needs and changes in condition, attending patient caseconference, communicating effectively to the patient, family, physician,other clinicians, agency management staff, and case coordinator,community case managers (as applicable) Ensures confidentiality ofpatient health information per agency policy.
