Job Overview
In response to collected data, implements, and evaluates nursing caredesigned to meet the individual health and illness needs of complex patientsand their families in collaboration with other disciplines. Carries outpatient responsibilities under direct supervision of registered nurse withemphasis on family-centered care.

Qualifications
Education: High School Diploma or GED and completion of a Practical Nursingcertification or diploma program.Experience: One (1) year of LPN experience.Certification(s): BLS from the American Heart Association / CPR.Licensure(s): State of Colorado Department of Regulatory Agencies(DORA) Licensed Practical Nurse (LPN)

Reports signsand symptoms of deviation from normal health.Contribute to the assessment of the patient's health status, report andrecord objective and subjective data to assist with carrying out the plan ofcaDevelops goals to advance clinical practice and professional growth through anunderstanding of the need for self directed personal and unit basedprofessional and educational activities.Participates in quality and process improvement initiatives.Provides support and assistance to members of the healthcare team tocoordinate the delivery of care.Under the supervision of the RN provides developmentally appropriate care,interventions and evaluates the effectiveness of interventions and communicateoutcomes of care given.
