　　Responsibilities:

　　Facilitate Personalized Plans of Care: Work closely with clients and their families to develop and implement individualized care plans that address their unique needs and preferences.

　　Perform Skilled Nursing Services: Provide expert nursing care, ensuring that clinical procedures and protocols are followed to maintain the highest standards of healthcare.

　　Collaborate with Interdisciplinary Teams: Engage in effective communication and collaboration with, therapists, and other healthcare professionals to enhance the overall well-being of our residents.

　　Qualifications:

　　Valid and current State of MN LPN license.

　　Proven experience in home care or a related healthcare setting.

　　Strong organizational and interpersonal skills.

　　Ability to develop and implement comprehensive care plans.
