Cuyuna Regional Medical Center * Diabetes Education Crosby, MN Nursing Full Time, 8-hour Day Shift FTE: 1.0 FTE (80 hours per pay period) Weekend Requirements: No weekends Posted 01/17/2024 Req # 12896 This position is an internal posting for an increase/decrease in hours for a particular CRMC employee to apply/sign (1/17-1/23) POSITION SUMMARY At the general direction of the Diabetes Education Coordinator, the diabetes assistant is responsible for the overall coordination of the diabetes education daily clinic. This includes assistance with patient assessment and education, appropriate documentation and serving as the communication link between the patient, diabetes educator, physician and other clinic departments or agencies involved in the patient's care. Complies with MN state law and the Nurse Practice Act. POSITION QUALIFICATIONS Education and Experience: One year of clinical nursing experience required. License/Certificates: Current Minnesota LPN License. American Heart Association BLS certification within 30 days of hire. Special Skills and Aptitudes: Must have passion to provide care for the patient living with diabetes, and to provide empathy to their unique situation. Effective observational, reporting, documentation and communication skills. Basic understanding of data collecting & reports. Knowledge of nursing prinicples, practices and techniques gained through a variety of nursing experience. Completion of specific education programs as recommended by the diabetes education department. Exemplifies self-direction with good organizational, analytical and interpersonal skills. Knowledge and compliance of state and federal accrediting agencies. ESSENTIAL RESPONSIBILITIES Direct Patient Care: Room and assess patients, anticipating appropriate type of visit for the patient/Diabetes Educator time. Assist the Diabetes Educator with 1:1 visits, group visits and supply resources and necessary. Operate correctly a variety of pieces of medical equipment including, but not limited to: blood glucose meters & lancets, tympanometer, scale, suction, oxygen, wheelchair, scooter, exam table, personal insulin pumps, Continuous Glucose Monitor hook-up & disconnect. Know and identify signs and symptoms of hypoglycemia & the treatment for it. Reinforce instructions to patients regarding education to prescribed therapies and behavior goals. Indirect Patient Care: Document vitals, treatments, interventions, medications, labwork and essential notes in the patient's medical record according to diabetes education policy/ADA criteria. Download various meters and insulin pumps and include appropriate information in the patient's chart. Stock supplies as needed in diabetes education office and common storage areas. Assist Diabetes Educators and scheduling staff in maintaining an accurate daily schedule providing efficient patient flow. Schedule lab tests or other appointments as directed by the diabetes educators & per policy. Call, fax or electronically send prescription refills as ordered by physician/CNS or standing order and document according to policy. Facilitate communication between patient and Diabetes Educator and provides general diabetes education information in a professional manner. Manage voicemail for self by recording appropriate greetings and checking voicemail. Take accurate and legible messages, task messages to diabetes educators for follow-up, prioritize calls and provide timely follow-up to patient. Identify and follow appropriate procedures for emergency calls. Problem solve and secure alternate solutions as necessary. Utilize excellent customer service skills in order to positively impact the experience of care for our patients, their families and other service partners.
