　　OUR Department:

　　Rochester Regional Health offers a full range of Dialysis treatment options to patients with chronic kidney disease. We treat adults from people from multiple backgrounds, cultures and ethnicities. We care for patients temporarily, either their kidney function recovers or they return to their home dialysis center, move or get a transplant; or some are with us on an ongoing basis. Patients have dialysis treatment anywhere from 2-3 days a week, from 3 to 5 hours each treatment. Our staffing model consists of RN's, LPN's and Techs.

　　As a Licensed Practical Nurse in Dialysis, you are key in the success of operations and compassionate patient care every day.

　　STATUS: Full-Time (40 hrs/week)

　　LOCATION: Bay Creek

　　DEPARTMENT: Dialysis

　　SCHEDULE: Monday-Saturday 6am-9:30pm

　　ATTRIBUTES

　　Graduation from an accredited School of Practical Nursing or equivalentStrong communication, customer service and interpersonal skillsWillingness and ability to work early morning shiftsFriendly, compassionate and patient focusedCompletion of the NYS Department of Health and NYS DOE approved training curriculum for LPN's working in Outpatient Hemodialysis Units (completed as part of unit based orientation)Completion of the NYS DOH and NYS DOE approved annual competency for LPN's accessing central lines in a hemodialysis unit (completed as part of unit based orientation)RESPONSIBILITIES

　　Patient Care & Evaluation. Conduct a thorough assessment of each patient's medical status upon admission and throughout the patient's course of treatment; provide regular patient status updatesTreatment Administration. Assist in the implementation of the patient's treatment plan under the direction of the Registered Nurse (RN); initiate and discontinue dialysis treatments using multiple access sites; administer IVP medications to dialysis patientsDocumentation. Complete required documentation in an accurate and timely manner Nurse, in accordance with established plans.PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

　　The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
