JOB REQUIREMENTS: Overview Performs nursing services that require theuse of physical, social sciences and nursing skills in the care of thesick, the conservation of health and the prevention of disease inpatients home. Responsible for the care of the patients assigned asdirected by the registered nurse. Other duties as assigned.Responsibilities Performs skilled nursing care and treatments as orderedby the physician and directed by the registered nurse. Teaches basicprinciples of palliative and restorative care as well as chronic diseasemanagement for all age groups; adapting and applying information to theneeds, interests and cultural patterns of the patient and family; guidespatients and families towards self-help in the recognition of physical,emotional and environmental health problems. Teaches and validatespatient and caregiver to assure safe delivery of care. Collects patientdata. Using nursing processes, identifies and reports changes incondition to the registered nurse. Maintains required records anddocumentation; uses them appropriately for the care of patients andfamilies. Documentation is submitted according to policy and orders arekept current. Assists with implementing a plan for continuity or carethat involves the patient, caregiver, other health care team members andcommunity resources. Organizes, prioritizes and plans daily patientassignments effectively. Demonstrates proper use of PPP, hand washingand bag technique to promote infection control and safety. Demonstratesunderstanding of safety in the community and work environment. Assumesaccountability for assigned equipment (both medical and computerequipment). Participates in quality improvement activities. Attendsappropriate continuing education in-services regularly and maintainsprofessional competencies set by department yearly. Education andExperience Graduate of an accredited School of Practical Nursing Aminimum one year relevant experience preferred. Certification/LicensureCurrent Illinois or Wisconsin licensure by the IL Depart of ProfessionalRegulation (IDPR) and/ or Wisconsin Department of Safety andProfessional Services required Approved CPR or BLS certificationrequired within 90 days of hire For those driving Mercyhealth vehicles:Maintain a valid driver\'s license.
