Summary: Licensed Practical Nurse (LPN) - Assumes responsibility oftotal resident care on a designated unit for each shift worked byperforming professional nursing duties and assuring the completion ofnurse aide duties. Cooperates with all disciplines to assure highquality resident care.
Duties and Responsibilities:
The LPN maintains charge and direction of floor in cooperation with theDirector of Resident Services (DRS) to assure quality resident care isprovided.
LPN creates nurse aide assignments if needed to meet the needs of theresident.
LPN Notifies DRS when a resident condition changes and when an issuearises with other staff members.
LPN administer medications and treatments in accordance with establishedpolicies and procedures.
LPN keeps medicine cart locked at all times when out of view.
LPN updates resident support plans on a daily basis as needed.
Appropriately assess residents and follow through according toestablished policies and procedures.
Assist with resident ADLs as necessary.
Assist with resident meals and fluid intake on a daily basis to assureproper nutrition is maintained.
Accompany physicians on resident visits as needed to assure continuityof care.
Transcribe physician orders per policy in a timely manner.
Observe nursing care and visit residents throughout the shift to assurethat nursing care is carried out as directed.
Participate in receiving report from off-going shift and giving reporton-coming shift to assure continuity of care. Communicate necessaryinformation to nurse aides.
LPN monitors care provided by Nurse Aides.
LPN completes resident incident/accident reports per policy in a timelymanner.
LPN collects samples, such as urine or sputum, from residents fortesting, as needed.
LPN communicates information as necessary with families, residents,staff, etc.
LPN assures that appropriate and adequate supplies are available onfloor for all disciplines.
LPN participates in staff meetings to provide and enhance communicationbetween all disciplines on all shifts.
LPN Participates in problem resolution regarding employees and otherfloor issues.
Participate in orientation and training of personnel.
Assure a clean, safe and orderly environment for residents, staff andvisitors.
Accurately document all care provided to resident during shift andbefore the end of the shift.
Chart medications immediately after administered, per policy.
Provide input to DRS concerning nurse aide performance.
Maintain confidentiality.
Attend and participate staff meetings, in-services, mandatorytrainings/in-services and continuing education programs.
Always functions within the limits of the Pennsylvania StateProfessional Standards Title 49 Chapter and in accordance with the scopeof the practice permitted with the licensure.
Performs other duties as assigned.
