　　QUALIFICATIONS:

　　Graduation from an accredited nursing program with a current NYS state licensure as a LPN.Direct experience in primary care (preferred but not required).Reliable transportation between sites required.Current CPR and Basic Life Support Certification (BLS).

　　JOB SUMMARY: Working with a diverse patient population, this position will provide relevant nursing and support services in a primary care setting to patients with acute and chronic illnesses. They are responsible for providing basic medical attention to patients and assessing their wellbeing as part of a larger medical team. Their duties include checking vital signs, recording a patient's medical history and other relevant tasks to support providers and care for patients.

　　Essential Job Duties:Provide direct patient care from birth on in a busy FQHCPediatric Primary CareAdult Primary CareOB/GYN CareDemonstrate knowledge of current standards of practice and perform current nursing practice expectations within established guidelines. Provide direct patient care as assigned and qualified to perform.Work collaboratively with clinician (MD, NP, PA, CNM, DO) to provide excellent clinical care.Perform medical treatments (medication and vaccine administration) as prescribed by a medical provider.

　　ADMINISTRATION:Collaborate to improve patient outcomes and provide quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction.Complete all patient charting and ensure all patient records are current/complete with strong attention to detail and accuracy.

　　PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.

　　TRAVEL REQUIREMENTS: Must be able to travel to sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
