Licensed Practical Nurse (LPN)
Maximus is currently looking for Licensed Practical Nurse (LPN)Assessors to join our growing team. As a Nurse Assessor, you willconduct person-centered assessments in accordance with contractdirectives, company processes, and contract requirements.
In this role, there is no direct hands-on care. We assess individuals inhomes, facilities, and group homes for long term care needs and facilityplacement.

*Must be able to commute into and throughout one to two counties in thestate of Maine for in home assessment completion.
Education and Experience Requirements\Essential Duties and Responsibilities:\* Responsible for completing medical and/or behavioral healthassessments within contract requirements\* Conducts on-site, video call and/or telephonic assessments todetermine an individuals needs for services and supports, eligibility,level of care or related outcome\* Perform all job duties in compliance with Person First standards,HIPAA guidelines, and company confidentiality policies and procedures\* Travel may be required based on program contract requirements
Required Skills/Abilities:\* Knowledge and understanding of medical and/or behavioral healthdiagnoses and prescribed medications\* Knowledge of community support programs, long-term care assessmentand level of care in medical, behavioral health or related programs,preferred\* Ability to collect data, define problems, establish facts, and drawvalid conclusions\* Excellent written and verbal communication skills\* Excellent interpersonal and customer service skills\* Proficient in Microsoft Office Suite\* Excellent organizational skills and attention to detail\* Ability to work in a fast-paced environment\* Ability to work independently and meet timelines
Education and Experience Requirements:\* LPN License in the state of Maine or hold a compact licensure\* One (1) year of clinical experience\* Experience in Long Term Care or Home Health Care preferred\* Electronic Charting System experience preferred\* Local travel is required to perform in-person assessments
Home Office:\Some work may be completed at home after field assessments\* Maximus will provide laptop, printer, and cellphone\* Reliable high-speed internet service\* Minimum 20 Mpbs download speeds/50 Mpbs for shared internetconnectivity\* Minimum 5 Mpbs upload speeds\* Private and secure workspace
