About Licensed Practical Nurse (Days & Evenings)

The Medication Reconciliation LPN is a certified medication expert who supports pharmacy and admission team services by obtaining medication and allergy histories for patients that will be admitted to the hospital. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.

  Responsibilities:

The LPN is prepared to function as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place.

The patient's care needs surpass the LPN's knowledge, skill or ability.
