Monthly one-to-one meetings with manager for support, professional development, and reflection
Duties:
Determines client service needs by assessing individual client's level of functioning, behavioral health symptoms and history
Determine diagnosis and treatment needs
Develop individualized client treatment plans and review frequently
At each session, determines the degree of Danger to Self (DTS) behavior or intention of client
If needed, coordinates the petitioning of a client for a court-ordered evaluation
After each session completes appropriate documentation
Coordinate and collaborate with all clinic support staff to assist clients with various needs
Provide and follow-up on referrals when appropriate
Completes discharge plan when clinically appropriate
Qualifications and Education Requirements:
Current, unrestricted license issued by the Arizona Board of Behavioral Health Examiners: LMSW, LAC, LPC, Pyh.D, or LCSW.
Must have a minimum of 1 year outpatient experience.

Pay Details: $50,336.00 to $95,000.00 per year
