　　Responsibilities

　　Provides behavioral health counseling services.

　　Conduct patient assessments and establish therapy treatment goals.

　　Administer individual and group therapy.

　　Maintain thorough and confidential records of observations.

　　Evaluate progress and effectiveness of therapy interventions.

　　Provide patient with information regarding resources and therapeutic activities in the community

　　Work directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge.

　　Requirements

　　Licensure / Certification / Registration:

　　One or more of the following:

　　Counselor credentialed from the Oklahoma State Board of Behavioral Health.

　　Social Worker specializing in Clinical Practice credentialed from the Oklahoma State Board ofLicensed Social Workers.

　　Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) required.

　　Preferred Credential(s):

　　BLS Provider preferred. American Heart Association or American Red Cross accepted.

　　Clinical Counselor preferred

　　Education:

　　High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.Additional Preferences

　　Ability to work in a collaborative care model with medical providers

　　Strong and timely written and verbal communication

　　Ability to manage own caseload from initiation to termination

　　Primary location in Broken Arrow with secondary location at another Tulsa area clinic
