　　We are currently seeking a full-time Social Worker support our residents.

　　Social Worker Responsibilities Include:

　　Interviewing participants and their families upon admission

　　Providing psychosocial assessment and counseling to residents and their families

　　Developing specific care plans based on the needs of individual residents

　　Helping residents apply for appropriate social services where qualified

　　Providing crisis intervention as needed

　　Providing grief and supportive counseling

　　Strengthening communications among residents, families, and the facility staff

　　Assisting with the discharge planning of residents along with interdisciplinary team

　　Social Worker Requirements Include:

　　Bachelor's Degree

　　Long Term Care exp. is a plus

　　NYS Screen Certification preferred

　　Strong admission and discharge planning experience
