Job Overview:This position provides medical case management for assigned populations - injury and illness. This position may assume the roles of education, administrator, researcher and consultant. The primary focus is disability case management utilizing highly developed critical thinking skills. The goals of the position may include: control and minimize the cost of the disability; promote optimal recovery and minimal lost work time; implement Disease Management programs for at-risk employees; provide effective counseling of disabled employees and appropriate referrrals and/or treatment alternatives; provide health care service coordination that promotes, improves and manages health care within the resources available; and collaborate with customers, clients and other healthcare team members to establish realistic goals and activities to guide both clients and providers in implementing care with the goal of successful return to work, consistent with physician recommendations and any applicable employer policies - i.e. modified duty, etc. - through communication with employees, physicians, department managers, and employee relations consultant. Position is remote but must live within driving distance of organization.Job Requirements:Associate's Degree in NursingHires into position (internal and external) required to obtain BSN within 5 years of hireRegistered NurseComputer programs - Word, Excel - to track disabled employees' progress, measure outcomes, and prioritize work flow STD/LTD/ADAAA/FMLACase management process and practiceCommunication and counseling skillsKnowledge of current health care practices5-7 years experience Professional in Nursing preferably in an Occupational Health or Acute care settingJob Responsibilities:Maintains compliance with HIPAA privacy requirements in the transfer of personal health information in all forms.Track injury/illness trends and provide accurate reports/information to management.Enroll "at risk" employees in disease management programs and monitor progress.Coordinate case management with other Employee Health Lifestyle Center functions and promotes the health and wellness focus of the department.Collects information about an employee's injury or illness to determine individual needs, and implements action plan to achieve recovery and safe, timely RTW. Maintains communication between employees, managers, and providers.Collaborates and coordinates the Modified Duty Program between employees and managers, and ensures placements meet employee restrictions.
