  Job Description:

  Under the direction from a physician, and within the established practice guidelines for an Advanced Practice Clinician, is responsible for performing a variety of duties in the evaluation, treatment plan and care, diagnosis and follow-up of assigned patients. May provide preventative, pre-operative and post-operative care.

  Provide clinical supervision and operational oversight of all departmental advanced practitioners and to work with administrative leadership to implement process and clinical improvement.

  Essential Duties & Responsibilities including but not limited to:

  As the lead Advanced Practice Clinician responsibilities include orientation, training and mentoring of newly hired Advanced Practitioners. In consultation with the Department Chair provide input to performance reviews. Attend regularly scheduled meetings with the department leadership team. Some off shift work hours and carrying a beeper as necessary.

  Non-Clinical Functions:

  Manages and communicates information through various processes including but not limited to use of computers, telephone systems, direct and indirect observations and documents in the medical record.

  Completes a quarterly review with the supervising Dept Chair on a timely basis.

  Complies with all policies and procedures as set forth by Lahey Clinic and all state and federal regulations.

  Meets all guidelines for continuing education/re-certification for continued practice in the department.

  Assists department in controlling expenses on an on-going basis and identifies areas of potential cost

  savings.

  Is alert to potential Quality Assurance/Continuous Quality Improvement (CQI) issues and actively participates in the problem solving process.

  Conducts advanced practitioner meetings

  Responsible for schedule and CME requests

  Monitors productivity

  Attends leadership meetings as assigned and shares relevant information the AP team

  Participates in interviewing and hiring of AP team

  Patient Care Assessment:

  Demonstrates necessary clinical skills to provide care for patients.

  Receives and triages telephone requests for information, appointments and/or medication renewals. Makes appropriate and timely decisions regarding the patient’s complaint and/or request, utilizing clinical knowledge and skill to extract pertinent information.

  Obtains detailed history and performs comprehensive physical examinations, completes medical histories, physical assessment and differentiates normal results from abnormal findings. Identifies wellness and risk factors: communicates relevant data via dictation and/or provides complete, accurate written documentation in the patient’s medical record. Determines and orders necessary laboratory and diagnostic tests. Interprets various diagnostic tests including blood tests, MRI, CT scans, EKGs, etc. Assists patients in areas other than the primary specialty including (but not limited to) recognizing the patient’s mental and physical conditions, and initiates appropriate assistance from other health care professionals.

  Anticipates and intervenes on potential problems/complications before they occur and initiates protocols and therapeutic measures in collaboration with the physician and according to established standards of practice.

  Patient Care Planning:

  Develops plan of care and reviews progress on an on-going basis.

  Initiates protocols and therapeutic measures in collaboration with physicians according to established standards of practice.

  Formulates desired outcomes specific to the individual patient problem/diagnosis and specifies interventions as appropriate.

  Prescribes, in collaboration with or at the direction of the attending physician, medications specific to the patient’s diagnosis and level of medication tolerance and in accordance with Lahey Clinic policies and procedures.

  Reviews the plan of care on an ongoing basis and revises the plan as needed per patient’s progress or lack of progress. In conjunction with the attending physician, researches medical protocols to determine an appropriate change in care plan.

  Identifies, coordinates and delegates as necessary, appropriate health care professionals to facilitate delivery of care. Provides appropriate and timely communication with the attending physician.

  Patient Care Implementation:

  Actively involves the patient and family as appropriate in implementing the plan of care. Prioritizes care needs and ensures the appropriate availability and accessibility of treatment for the patient.

  Utilizes physical assessment skills on an ongoing basis to identify, communicate and document patient problems. Recognizes changes in mental and physical condition. Implements appropriate action based on assessment to ensure patient safety, well being and satisfaction.

  Anticipates potential emergency situations and performs appropriate treatment/intervention measures. Performs a variety of routine procedures to expedite patient’s response to the plan of care.

  Patient Care Evaluation:

  Participates in grand rounds as necessary to ensure continuity and quality of care.

  Follows through on an achievement or lack thereof based on outcome evaluation. Provides appropriate and timely communication to attending physician.

  Patient Care Documentation:

  Provides thorough and accurate documentation in the medical record by documenting all pertinent data, therapeutic interventions, patient responses and any medical/surgical procedures.

  Ensures that summaries include complete examinations, office visits, telephone conversations and testing requests.

  Ensures that all documentation is completed in a timely manner per Lahey Clinic policy.

  Organizational Requirements:

  Maintains strict adherence to the Lahey Hospital and Medical Center Confidentiality Policy.

  Incorporates Lahey Hospital and Medical Center Guiding Principles , Mission Statement and Goals into daily activities.

  Complies with all Lahey Hospital and Medical Center Policies. Complies with behavioral expectations of the department and Lahey Hospital and Medical Center.

  Maintains courteous and effective interactions with colleagues and patients.

  Demonstrates an understanding of the job description, performance expectations, and competency assessment.

  Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.

  Participates in departmental and/or interdepartmental quality improvement activities.

  Participates in and successfully completes Mandatory Education.

  Performs all other duties as needed or directed to meet the needs of the department.

  Qualifications :

  Graduate of a Master’s Program in Nursing

  Certification and current NP/PA license to practice in the Commonwealth of Massachusetts
