A successful candidate must be committed to excellence, delivering comprehensive primary care in an outpatient setting at our UK Turfland location. With several clinics in the state, we serve as leaders at UK HealthCare, a $4 billion health system, in its primary care delivery strategy by implementing value-based clinical redesign and population medicine programs. We have aligned our clinical approach to enhance value-based quality measures through pre-visit planning, comprehensive nurse assessment, and a UK HealthCare Population Health team involving social workers, community health workers, and RN care managers. We are a national leader in transitional care management and its impact on unnecessary hospital readmissions.
