The HVI Care Coordinator has coordination and clinical skills required for the comprehensive management of assigned. HVI patients in area of specialty. This position assists the team and coordinates involvement of other specialty teams necessary to develop a comprehensive plan for the patient, and reviews the plan of care with the other members of the clinical team, participates in patient care, organizes procedures/services, and coordinates discharge planning (as applicable). This position may participate in the education of employees, residents and medical/nursing students.
MINIMUM QUALIFICATIONS:EDUCATION, CERTIFICATION, AND/OR LICENSURE:1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).2. Obtain certification in Basic Life Support within 30 days of hire.EXPERIENCE:1. Two years clinical experience in nursing.PREFERRED QUALIFICATIONS:EDUCATION, CERTIFICATION, AND/OR LICENSURE:1.Certification in area of specialty.EXPERIENCE:1.
