Purpose
To provide housing case management to HUD assisted clients seeking housing and services in the New Haven region. This position will primarily support clients in the housing application process, housing search, and independent living with the ultimate goal of self-sufficiency and housing stabilization.
Duties and Responsibilities
Assess needs and strengths to develop clients goals.
Assist with identification and documentation for securing Basic Needs and entitlements such as Medicaid, Supplemental SS, SSDI, Nutrition Assistance, housing subsidies etc.
Help identify, secure and occupy safe and affordable housing.
Assist with budgeting, bill-paying, housekeeping, shopping and cooking.
Help integrate mental health and addiction treatment and use of medical and wellness services.
